Diabetes had been a significant problem globally as it leads to premature death and multiple complications like amputation, cardiovascular diseases, and blindness to patients. Type 2 diabetes is a principal non-communicable health problem in the world (Green et al., 2017). It is difficult and expensive to treat and manage. Risk factors are very predominant; for instance, genetic susceptibility and environmental factors seem to be responsible for the development of severe conditions. Diabetes is affecting the middle-aged adults, and the younger population who tend to be a peak at economic productivity, and the overall cost associated with management and prevention is considered to be significant. Hence prevention and management of type 2 diabetes is a perfect capstone project topic to be considered.
Type 2 diabetes can be prevented through lifestyle interventions, medications, and metabolic surgery. However, the education and knowledge about the tools of intervention tend to be limited among the primary care professionals, and therefore, more knowledge should be imparted on the care physicians. Quality initiative measures will be a major goal of the change proposal. Performance measures must assess the level and extent of care accorded to diabetes type 2 populations across the world. Diabetes type 2 is a complex medical issue that can be addressed by professional team members, and training of the health care providers must be undertaken since the team can effectively help many people so that they can easily cope up with the arising complications like kidney disease and macro vascular complications. For example, it's evident that when a caring clinician collaborates with pharmacists to develop individualized medication and advising the patient on medical usage, it can improve intervention mechanisms.

Diabetes management and prevention are significant to maintaining a quality life and deterring long term complications, and it occurs daily in the context of interpersonal interactions. Social interactions are central to meeting the demands of managing and preventing diabetes type 2. The management's social context includes family, peers, health care providers, and romantic partners. Diabetes management is most effective when it happens in the context of the support of close relationships. Individuals must seek support from the social world during major transitional developments.

There exists a wider range of diabetes type 2 management and prevention strategies in the current world. Identification of interventions that seem to be substantively effective should be used. Also, lower-cost professionals like nurses, pharmacists, and physicians can play a critical role in delivering medication adherence education and lifestyle prevention measures to the patients (Smiley et al., 2016). Pharmacist’s interventions must be promoted due to strong efficacy and impact on glycemic control as they also tend to be cost-effective and easily accessible to healthcare workers.
Effects of the Problem
Diabetes issues can have a greater impact on the body that may be long term in nature. It affects the blood vessels and nerves. Also, it can result in diabetic retinopathy due to poorly controlled diabetes as the blood vessel sat the back of the eye swell and start leaking. High blood pressure is expected and linked to high cholesterol, which has to increase heart attacks and cardiovascular disease (Herman, 2016).
Significance of the Topic
The topic is very important as it offers excellent training and quality assurance to the health care providers on prevention and management mechanisms for diabetes to people in the community. It influences the nursing practice as the nurses play a critical role since they often carry out annual diabetes checks and clinical roles in screening, maintaining, and supporting diabetes patients (Härmä-Rodriquez, 2016).
Prevention and management of type 2 diabetes by changing the dietary pattern and lifestyle
Type 2 diabetes commonly starts in the mid-50s and is not inevitable. However, changes in lifestyle can significantly reduce the chances of an individual getting complications. Therefore, it requires that dietary habits developed (Basterra et al., 2017). With proper testing, treatment, and lifestyle must be changed, the healthy eating strategy implemented, promotion of physical exercise to promote the management, and prevention of diabetes (Walker et al., 2018).

Diet is an essential factor in the diabetic condition's management, so the patient and healthcare provider must understand the dietary needs of the patients.
