A case formulation is a tentative way which is adapted to help clients to understand their psychological problems, their origin, and factors that keep them going. Cognitive Behavior Therapy (CBT) case conceptions have follows a 5p's model. The essay will describe Greg's case study using 5P's model. Greg experiences feelings of stress and anxiety over the past six months. This condition did not develop six months ago but developed during childhood when he failed to meet his father's expectations. These 5p's includes the presentation of problem, predisposition factors which made the Greg vulnerable to his condition. The factors that caused the problem are referred to as the precipitating elements. The model includes the perpetuating elements that involve the unintentional consequences of an attempt to deal with the condition or mechanisms which keep the condition going. Protective factors are the coping strategies the client can adapt to deal with stress and anxiety.

Formulation

Presenting problems

The presentation of issues affecting Greg is essential to the development and understating of his condition. Greg presents depression as a result of stress and anxiety. He has had three sessions of counseling where he revealed that he gets anxious and stressed when socializing with unfamiliar individuals. His heart beats rapidly and he deals with the situation by not socializing at all or leaving. For example, he admitted that recently left his friend's birthday party because he could not socialize with strangers. His problem had begun during childhood following his father's high expectations. He was not good and this disappointed his father silently. In his sessions, Greg says that "You could just tell by the look on his face that he was disappointed." He also hated his country and moved to the city when he turned 18 years. He felt left out in his family because that he "never fit in - I hate sports and they are all sports nuts." The country people focused on sport and community events which he was not a fun. Greg works as a supervisor in a bank and has never been promoted. He feels like he deserves a managerial position but has had difficulty progressing because he cannot express his capability. His colleagues who have been working for the same company are now in managerial levels.

He has had little experience of romantic relationships because he is identified as gay. Although Greg made some good friends at university and continues to see them regularly, he felt his reserved nature made it difficult to date. He has had a few relationships that have lasted a few months each. He wishes to be in a long-term relationship but the thought of dating is stressful. Despite disliking team sports, he enjoys being physically fit.

Precipitating factors

The subsequent stage of the case formulation includes articulating the external and internal issues that cause the presenting conditions. Individuals' life experiences develop dysfunction assumptions and beliefs about themselves and the world around them. They are activated by occurrences once this belief is triggered it produces a negative automatic thinking. Using the ABC model to understand the precipitating factors that trigger the include A- activating occurrence, B for Beliefs and C for consequences.

At a young age, Greg was disappointed when he failed to meet his father's demands.

His problem had begun during childhood following his father's high expectations. He was not good and this disappointed his father silently. He does not fit into the family because they love sports. They are country people focused on sport and community events which he was not a fun. Greg works as a supervisor in a bank and has never been promoted.

Perpetuating factors

Although the ABC model is the functional device, it has its restrictions in terms of helping to understand Cara's emotional disorders. He feels like he deserves a managerial position but has had difficulty progressing because he cannot express his capability. His colleagues who have been working for the same company are now in managerial levels. He is not able to build strong romantic relationships because he is identified as gay. Although Greg made some good friends at university and continues to see them often, he is unable to date. He blames himself for everything that happens in his life for instance when he fails to pass an interview or for a managerial position he criticizes himself and refers to himself as a fool.

Predisposing factors

It may not be clear what causes continue to cause the problem. One of them is lack of confidence making him unable to express himself and his ability to his employer. He does not forgive himself for disappointing his father. He also blames himself for every negative event in his life.

Protective Factors

Greg main strength is the determination he has for achieving a managerial level in a big company. He is able to express his problems during therapy sessions that help him to heal from his past experiences. He had started to seek for the same positions in other company before he failed his first interview. Greg should be encouraged that it is possible to achieve his carrier goal if he improves on his self-esteem.
