The project is to design an interactive educational program for type 2 diabetic patients. The program allows for self-management where the aim is to improve knowledge, encourage active participation of patients and provide supportive tools for management. The program will consist of 4-5 group sessions per week and 6-7 one on sessions with patients according to their needs.

Relevant professional nursing practices include soft skills and proper shift scheduling. Qualifications are essential to put in the right efforts where they are needed. Scheduling shifts will help to ensure that tasks are not boring for the nurses and that they can stick to comfortable hours of working (Fonseca, Bertolin, Gubert, & da Silva, 2019). Since everyone is aware of their tasks, they will maintain high-level competencies hence inter-professional collaboration.

The leadership strategies will change the care setting by ensuring that only the best practices are applied in the intervention. If it is a case of infection control, nurses play a significant role in helping the prevention of illnesses and adhering to the policies. There will be the utility of high skills, and the patient receives care from skilled staff members. Team nursing will also be an excellent delivery method as the unit gets divided into smaller groups where each staff member performs the duties they are best prepared (Schmitt, Gahr, Hermanns, Kulzer, Huber, & Haak, 2013). The delivery methods will improve the quality of the project because they will ensure that tasks are completed in time and that the model is adequate.
The technological options in improving delivery methods include automated pumps to control dosages and drips that nurses give to patients. This form of technology will allow nurses to change the amounts of drips easily and accurately (Kartal & Yazici, 2017). The technique of portable monitors is also included in improving delivery because they will be useful in allowing the nursing professionals to check up on the patients even when they are busy elsewhere (McCabe & Timmins, 2016). The delivery will be more effective because constant monitoring means that nurses will get alarm notifications in the case of an emergency. Emerging technology options include the use of wearable devices that will help in tracking heart rates, sleep, exercise, and respiration to help people take their health into their hands.
Week 3

The relevant stakeholders during the implementation of the intervention plan are the registered nurses, nurse managers, and patients. The needs of the stakeholders will have to be met for the success of the intervention. There is a need to satisfy the needs of the nurses so that they do a commendable job on the patients.
The healthcare regulations include the focus on purpose, such as providing terms of reference for effective working for quality and standards (Slowther, Johnston, Goodall, & Hope, 2004). The committee will be charged with focusing on quality and adhering to standards. The health care regulations will ensure the creation of a renewable work plan that supports the delivery of quality services. Other support considerations for implementing the program include the need for training and nursing education. The method requires competency-based education, which means that one can take their work experience and use it to administer their knowledge and expertise.
Policy considerations that would support the implementation are such as designing reports to ensure that there are clear reviews and monitoring. Reports will help to ascertain that operational activities were conducted rightfully. They will also retain all the overall responsibility and accountability for healthcare provision. The committee will also ensure to review the terms of references annually for follow-up on the operations (Devine et al., 2015). The new policy considerations will impact positively on the efforts of implementation because it will help promote quality services in health institutions. It will ensure the advocacy to embrace auditing and adherence to standards.

The timeline to implement an intervention plan will be three months. There will be a chronological order of events from the strategies that will be applied. The managers will build an accurate timeline to show the phases already in the past and the progress as well as the future (Keating, 2014). This way, there will be an appropriate projection of tracks for the intervention plan. Care for patients requires quick adjustment; hence nurses will have to fit in the schedule perfectly for the span of three months. The time frame is quite realistic because there is a clear definition of the project requirements. This way, the concerned members will follow a work breakdown structure that identifies the dependencies between the tasks and skillsets.
The factors that will impact the timing of implementation include the project scale and scope. Large organizations will require more time to develop. Complex sites also require more weeks of planning and execution. The project management will also be a factor to affect because it is dependent on experience where managers are to prevent and handle problems proactively. The existing health care policy will determine the need for changes and interventions. Relevant health care laws provide for the minimum and maximums.
In conclusion, health care is a delicate matter, and intervention plans should follow an order for success. The changes after implementation will impact the quality of care by ensuring a control over the performance measurements. Patients will experience the best care because of innovative practices that favor them. Nurses should be well-skilled for the work as their competency is needed to attain success.
Each session: set the goal at the end to motivate members to keep track of their meals, carbs intake, exercise and management of their diabetes.
