Introduction
The project will be completed at the United States Department of Veterans Affairs, Home Based Care Department at (Insert City, State) United States. The US Department of Veterans Affairs (VA) is a federal cabinet-level department that offers comprehensive care services to military veterans through the VA medical institutions and outpatient clinics throughout the US (Geriatrics and Extended Care, n.d.). The home-based care department refers to these services' provisions at the veteran's homes, where the nurses provide case management (USAGov., 2019). The case management include in-home support for persistent diseases and any illnesses that interrupt the veterans’ daily activities.
Goals
Through this project, my aim is to get practical experience with a vulnerable population that will equip me with valid, relevant, and reliable knowledge of healthcare provision, problem-solving, and communication skills.
Learning Objectives
Demonstrate my learned skills in caring for the vulnerable population, improve my teamwork capability, develop communication skills, and demonstrate my problem-solving skills when dealing with my colleagues, and the patients, identify the limitations within the United States Department of Veterans Affairs, Home Based Care Department, and offer practical solutions.
Justification
Wars expose soldiers to many mental, physical, and social issues. Combined with old age, the veterans are some of the most vulnerable populations. Some of the problems they face include brain damage, depression, substance abuse, homelessness, and trouble adjusting to civilian lives (Nolfi, n.d.). These are the risk factors for conditions such as ulcers and other terminal conditions. As such, they need special care from both the professionals and society, forming the basis for this project. My unit focuses on keeping the standards and improvement in the veteran homecare to provide practical preventative measures on ulcers, depression, controlled tobacco, alcohol use, and medication management.
Conclusion
With the current COVID-19 pandemic, the team will also reduce this population's vulnerability by making the veterans' home environment safe, reducing hospitalization, and making the family members as involved as possible in the care department.
